Social Welfare Programs

At Hirondelle, we provide welfare services for patients in consultant with their families and the multi-disciplinary team. Our Social welfare officers work with patients and their families to ascertain the needs and goals of the patient or veteran, facilitating the ongoing continuity of care from the hospital in to the community and within the home or other facility.

On the completion of a rehabilitation program at Hirondelle, the Social welfare team can assist in organising services for those patients who are identified as needing these services to enable their return to the community. The services organised for patients by Hirondelle may include community funded programs such as home care, meals on wheels, community nursing and transition from hospital to home programs.
